1.CÁC XUNG CHỤP NHANH.
Chuỗi xung FSE (fast spin echo) có tên gọi khác là TSE (turbo spin echo), đây là chuỗi xung SE cơ bản nhưng được cải tiến để rút ngắn đáng kể thời gian tạo ảnh. Trong FSE, trong mỗi chu kỳ tạo ảnh (TR) thay vì chỉ có 1 người ta sử dụng nhiều xung FR 180° liên tiếp nhau. Đồng thời cũng kích hoạt hệ thống chênh từ mã hóa pha (phase encoding gradient) tức Gradient Y ngay trước mỗi echo.
Kết quả là trong cùng một TR, có nhiều echo được tạo ra sau mỗi xung kích thích và mỗi lần chênh từ mã hóa pha khác nhau. Chuỗi các tín hiệu echo được tạo ra trong một TR gọi là một “echo train” và số lượng echo thu được gọi là ETL (echo train length).
So với SE cơ bản, FSE không chỉ đơn thuần rút ngắn thời gian tạo ảnh mà thực sự FSE có những đặc điểm khác biệt riêng:
1.1. Xung Fast SE
Sau mỗi TR, SE cơ bản chỉ thu được 1 echo còn FSE thu được nhiều echo. Để có thể thực hiện được nhiều xung đảo ngược RF 180, TR trong FSE cần kéo dài hơn, thường là 4000ms hoặc hơn (trong SE cơ bản có TR khoảng 2000-2500).
FSE sẽ có lợi thế trong tạo hình ảnh T2W khi cả TR và TE đều kéo dài.
FSE cũng không giống với kỹ thuật multiecho trong SE cơ bản, do ngoài việc sử dụng nhiều sóng đảo ngược RF 180, FSE còn sử dụng cả Gradient chênh từ Y để tạo echo.
FSE còn có khả năng cho phép thực hiện kỹ thuật tạo ảnh “double echo” bằng cách chia echo train ra làm đôi. Ví dụ trong một TR, có “8 echo trong một echo train”, thì 4 echo đầu được sử dụng để tạo ảnh PD-weighted và còn 4 echo sau được sử dụng để tạo ảnh T2-weighted.
1.2. Xung HASTE
HASTE (half-Fourier acquisition single shot fast spine echo) còn có tên gọi khác là SSFSE (single-shot fast spin echo). Đây là những kỹ thuật tạo ảnh cộng hưởng từ siêu nhanh (ultrafast), có thời gian cắt khoảng 1 giây hoặc dưới 1 giây.
Nếu như trong FSE dựa vào RF và các Gradient để rút ngắn thời gian tạo ảnh thì HASTE lại dựa vào nguyên lý mã hóa tín hiệu trên k-space và thuật toán biến đổi Fourier. Cụm từ “single-shot” trong HASTE có nghĩa là chỉ bằng 1 xung RF kích thích thì một nửa số đường trong k-space được lấp đầy, và do vậy biến đổi Fourier cũng chỉ cần bán phần (half-Fourier). TE trong HASE kéo dài, do vậy chuỗi xung này thượng được ưu tiên ứng dụng cho các mô có TE dài, ví dụ như các khoang chứa dịch tự do (túi mật, bàng quang, khoang dưới nhện, ổ áp xe, dịch cổ chướng, dịch khoang màng phổi…). Một số ứng dụng hay gặp của HASTE là chụp đường mật – ống tụy (MRCP – MR cholangiopancreatography), chụp tủy sống (MR myelography), chụp đường bài xuất hệ tiết niệu (MR urography).

1.3. Xung Fast GRE
Bản thân chuỗi xung GRE đã là một chuỗi xung nhanh, tuy vậy trong thực tiễn lâm sàng vẫn đòi hỏi có những chuỗi xung siêu nhanh (ultrafast) mà vẫn tận dụng được những ưu điểm của chuỗi xung GRE. Fast GRE hay còn được gọi là Turbo GRE có thời gian cắt lớp không quá 5 giây, thường chỉ 2-3 giây mà chất lượng hình ảnh vẫn rất tốt.
Các chuỗi xung GRE được cải tiến chủ yếu dựa trên sự tác động vào k-space và thuật toán biến đổi Fourier (tương tự như HASTE). Ngoài ra, Fast GRE cũng sử dụng góc lệch nhỏ (<45) để tăng tương phản T1-weighted, tránh trạng thái bão hòa (saturation) do TR quá ngắn.
1.4. Xung EPI
EPI (echo planar imaging) là những chuỗi xung có thời gian tạo ảnh siêu nhanh, lớp cắt mỏng, rất ưu thế trong các chương trình chụp động học (dynamic) và cộng hưởng từ chức năng (functioning MR).
Một số ứng dụng thường gặp của EPI trong thực hành là MR diffusion (cộng hưởng từ khuếch tán), MR perfusion (cộng hưởng từ tưới máu), MR diffusion tractography (chụp đường dẫn truyền thần kinh trung ương), MR spectroscopy (cộng hưởng từ phổ). Tuy vậy EPI có một số nhược điểm:
Do sự biến đổi Gradient X, Y quá nhanh và liên tục nên hình ảnh do EPI tạo ra không nét và chi tiết như trên các chuỗi xung khác, có thể bị biến dạng so với hình ảnh thật.
EPI cần một bộ khuếch đại rất mạnh để có thể làm cho các Gradient đảo ngược nhanh và liên tục.
Cũng do sự biến đổi các Gradient X, Y quá nhanh và liên tục ở cường độ cao nên các chuỗi xung EPI có khả năng tạo ra những kích thích thần kinh cho người bệnh.
EPI rất ồn, gây khó chịu và căng thẳng cho người bệnh.
Khi chỉ sử dụng một kích thích TR (single-shot) độ tương phản của hình ảnh không cao và do vậy không có tương phản của T1.
1.5. Xung GRASE
GRASE (gradient and spin echo) thực là chuỗi xung kết hợp giữa fast-SE và EPI bằng kỹ thuật hybrid. Tín hiệu của fast-SE và EPI được tạo ra và thu nhận luân phiên nhau trong một chu kỳ tạo ảnh. Một loạt các xung RF 180 được sử dụng để tạo ra nhiều tín hiệu echo giống như trong FSE. Thêm vào đó thực hiện đảo pha nhanh – liên tục các Gradient-X, Gradient-Y (readout gradients) sao cho có nhiều GRE được tạo ra cho mỗi SE. Kết quả là tín hiệu, chất lượng hình ảnh và tương phản T2-weighted của xung GRASE thu được tương tự như SE nhưng thời gian cắt lớp còn nhanh hơn cả FSE. FSE là một chuỗi xung nhanh, thời gian cắt lớp ngắn hơn SE nhưng lại có nhược điểm là giảm độ tương phản và khả năng phát hiện các tổn thương chảy máu, vôi hóa. Chuỗi xung GRASE tận dụng được ưu thế cắt nhanh của EPI và tương phản của SE, đồng thời khắc phục được nhược điểm của FSE.
a. Hình ảnh T2W GRASE bộc lộ tổn thương dưới vỏ thùy trán trái (mũi tên), với độ tương phản rõ nét, tương đương với T2W SE.
b. So sánh với tương phản T2W FSE. Tổn thương dễ bị bỏ sót.
2.CÁC CHUỖI XUNG BÃO HÒA MỠ.
2.1. Chemical-shift
Trước hết cần hiểu rõ hiện tượng chemical-shift là gì? Trong cơ thể, các proton có thể ở trạng thái tự do như trong nước tiểu, dịch não tủy hoặc trạng thái liên kết với các phân tử như trong mô mỡ, mô cơ.
Ở trạng thái tự do, tần số cộng hưởng Larmor của các hạt nhân nguyên tử hydro (proton) chỉ phụ thuộc vào từ trường tĩnh Bo và hằng số hồi chuyển từ (o = Bo.Y). Khi ở trạng thái liên kết, do sự tương tác với các phân tử thì hằng số hồi chuyển từ Y của proton sẽ có một chút thay đổi, dẫn đến tần số cộng hưởng của các proton liên kết này sẽ thay đổi, mức độ thay đổi phụ phuộc vào vị trí và bản chất của phân tử liên kết. Sự khác biệt tần số cộng hưởng của các hạt nhân nguyên tử của cùng một nguyên tố do sự liên kết phân tử gọi là hiện tượng Chemical-shift. Mức độ khác biệt này cũng phụ thuộc vào từ trường tĩnh Bo và bản chất của phân tử liên kết. Đơn vị tính của chemical-shift là “ppm” – parts per million.
Trong cơ thể, hiện tượng cheimcal-shift xảy ra rõ rệt nhất giữa các proton gắn kết trong mô mỡ bởi các acid béo (triglyceride) và các proton tự do trong nước. Chemical shift của mỡ và nước là 3.5 ppm tại từ trường 1.5T, khi đó tần số cộng hưởng của các proton trong mô mỡ sẽ chậm hơn 225 Hz so với các proton tự do trong nước. Ý nghĩa của từ “chemical”trong cụm từ chemical-shift là đề cập đến sự khác biệt về cấu trúc hóa học của proton gắn kết trong mỡ (-CH2) và proton tự do trong nước (H2O) đã dẫn đến sự khác biệt về tần số cộng hưởng.
Chemical-shift giữa mô mỡ và nước. Trong từ trường 1.5T, tần số cộng hưởng của các proton gắn kết trong mô mỡ chậm hơn tần số cộng hưởng của các proton tự do trong nước khoảng 225 Hz, tại vị trí phân cách khoảng 3.5 ppm.
a. Tại thời điểm TE=0, Mxy của mỡ và nước inphase với nhau trong mặt phẳng-xy
b. Khi TE=2,2ms, Mxy của mỡ và nước oppose-phase (đối pha) với nhau. Tín hiệu thu được chỉ có của nước
c. Khi TE=4,2ms, Mxy của mỡ và nước trở lại in-phase (cùng pha) với nhau. Tín hiệu thu được gồm cả mỡ và nước.

Tại mặt phân cách giữa các tạng (ví dụ bao gan), trên ảnh opposed-phase có viền đen giảm tín hiệu bao quanh do có chemical-shift.Tín hiệu của nhu mô gan không thay đổi trên inphase và opposed phase do không có rối loạn bệnh lý chứa mỡ khu trú hoặc lan tỏa.
Nếu mỡ và nước trong cùng một voxel (đơn vị thể tích) thì sự khác biệt về tần số tiến động sẽ càng rõ rệt hơn khi thư duỗi T2 diễn ra, tức là các spin sẽ có 2 trạng thái là in phase (đồng pha) và out of phase (lệch pha), và nếu trạng thái out of phase diễn ra ở 1800 thì gọi là opposed-phase (đổi pha). Trong từ trường 1.5T, các proton trong mỡ và nước sẽ có trạng thái opposed-phase khi TE=2,2ms rồi trở lại trạng thái in-phase khi TE=4,2ms. Sau khoảng thời gian TE=2,2mm tiếp theo, các proton này sẽ lại ở trạng thái in-phase, quá trình cứ tiếp diễn liên tục như vậy được gọi là sự chuyển pha theo thời gian (time dependent phase shift).
Khi tạo ảnh cộng hưởng từ được thực hiện trong trạng thái in-phase, thành phần Mxy của các proton trong mô mỡ và nước sẽ cùng tạo echo có cường độ như nhau, kết quả là hình ảnh thu được là giống nhau. Trong trạng thái opposed-phase, echo được tạo ra từ các proton trong nước khác biệt với echo được tạo ra từ các proton trong mỡ, do vậy hình ảnh thu được là khác nhau. So sánh cường độ tín hiệu thu được giữa hình ảnh in-phase và oppose-phase sẽ cho biết mức độ có mặt của mô mỡ trong cơ quan cần thăm khám. Cụ thể là nếu một tổn thương có chứa mỡ, tín hiệu của mỡ trong tổn thương sẽ giảm tín hiệu trên hình ảnh oppose-phase so với hình ảnh in-phase.
a. Hình ảnh CTscanner không tiêm thuốc cản quang. Nhu mô gan giảm tỷ trọng so với lách do nhiễm mỡ.
b. Hình ảnh MRI In-phase. Cường độ tín hiệu thu được trong nhu mô gan mạnh (tăng tín hiệu).
c. Hình ảnh opposed-phase thấy tín hiệu của nhu mô gan giảm rõ rệt so với in-phase. Mô mỡ dưới da và khoang phúc mạc không bị giảm.
Cần chú ý rằng, chuỗi xung Chemical-shift chỉ thực hiện được đối với những mô có cả 2 thành phần là mỡ và nước. Còn nếu chỉ có mỡ hay gọi là mỡ đại thể như mô mỡ dưới da, các khối u mỡ (lipoma) thì chuỗi xung này không có tác dụng, tức là tín hiệu mỡ không bị bão hòa.
2.2. Xung STIR
Chuỗi xung STIR đã được đề cập trong phần các chuỗi xung phục hồi đảo ngược (inversion recovery) cùng với các chuỗi xung IR và FLAIR. Điểm chú ý ở đây là khác với chuỗi xung Chemical-shift, STIR được ứng dụng để bão hòa các tổ chức mỡ, không phụ thuộc vào sự có mặt của nước.
2.3. Xung SPIR
SPIR (spectral presaturation with inversion recovery) về bản chất không phải là một xung thực sự, mà chính xác chỉ là một module được áp dụng vào các chuỗi xung chuỗi khác để có hiệu quả bão hòa mỡ, ví dụ T1W FS, T2W FS (fat-supressed). Các chuỗi xung SPIR cũng làm đảo ngược Mo trước khi kích thích RF tương tự như STIR, nhưng SPIR không dùng xung đảo ngược RF180° như STIR mà SPIR đảo ngược chênh từ Gradient-Y để tạo ra sự đảo ngược Mo của mỡ và nước. Nói cụ thể hơn, SPIR là kỹ thuật áp dụng phương pháp kích thích nước (water-excitation), chỉ có mô nào có chứa nước thì mới có Mxy để tạo tín hiệu echo. Mô nào càng có nhiều nước thì càng có nhiều tín hiệu.
U mỡ (lipoma) trong phần mềm vùng gan bàn tay. T1W SE cornal (a) và T1W SE axial (b) thấy khối u tăng tín hiệu tương đương tổ chức mỡ dưới da, ranh giới rõ nét. T2W FSE FS (c) là chuỗi xung T2W Fast SE được kết hợp với kỹ thuật bão hòa mỡ FS cho thấy mỡ trong khối u và mỡ dưới da giảm tín hiệu rõ nét.
a. Hình ảnh CLVT u quái gan. Tổ chức mỡ trong khối u giảm tỷ trọng tương đương tỷ trọng mỡ dưới da.
b. Hình ảnh opposed-phase. Tín hiệu của tổ chức mỡ này và tổ chức mỡ dưới da không bị xóa.
c. Hình ảnh T1W FS (Gd).
Mỡ trong khối u và mỡ dưới da đã được xóa hoàn toàn.
Trên hình ảnh T1W và T2W, tổ chức mỡ tăng tín hiệu do thư duỗi T1 của mỡ ngắn (260ms tại 1.5T) do vậy sẽ rất khó có thể phân biệt được mô mỡ với dịch hoặc mô mỡ với tình trạng ngấm thuốc đối quang từ của tổ chức. Kỹ thuật SPIR được kết hợp vào các chuỗi i xung tương phản T1-weighted và T2-weighted để khắc phục vấn đề này.
3. CÁC CHUỖI XUNG MẠCH MÁU.
3.1. DARK – BLOOD
Minh họa nguyên lý tạo ảnh black-blood theo sự biến đổi của vector từ trường của các spin trong lòng mạch (mũi tên đen). Từ trạng thái tĩnh ban đầu, dùng một sóng đảo ngược RF180° không chọn lọc lớp cắt (nonselective) kích thích toàn bộ các spin trong vùng thăm khám. Tiếp theo đó sử dụng tiếp một sóng đảo ngược RF180° chọn lọc (slice-selective), chỉ kích thích vào các spin trong lớp cắt cần khảo sát, khi đó do dòng chảy tạo ra sự thay thế liên tục các spin nên cường độ tín hiệu trong lòng mạch sẽ lẫn lộn và triệt tiêu nhau. Đến thời điểm giữa tâm trương, thì sử dụng một chuỗi các xung Fast SE liên tiếp nhau để thu tín hiệu tại thời điểm cuối tâm thu, lúc này hầu như không có tín hiệu nào trong lòng mạch.
Minh họa nguyên lý tạo ảnh black-blood theo chu chuyển tim trong khoảng RR trên điện tâm đồ (ECG). Sóng RF180° không chọn lọc (NS) kích thích vào thời điểm cuối tâm trương. Tiếp theo đó sử dụng tiếp một sóng RF180° chọn lọc (S). Đến thời điểm giữa tâm trương, thì sử dụng một chuỗi các xung Fast SE liên tiếp nhau và thu tín hiệu tại thời điểm cuối tâm thu.
Dark-blood được thực hành như sau: thứ nhất dùng một sóng đảo ngược RF180 độ không sắm lọc, kích thích toàn bộ các spin trong lòng mạch và ngoài lòng mạch .Tiếp sau đó, dùng một sóng đảo ngược RF180 độ khác, mang tần số sắm lọc sở hữu lớp cắt phải khảo sát. Như vậy, do mẫu chảy liên tục trong lòng mạch, các spin mới sẽ thay thế các spin cũ bởi vậy tạo ra 1 sự hỗn loàn về chiều và hướng của những spin trong lòng mạch, làm cho cho vector từ hóa dọc Mz bị triệt tiêu lẫn nhau. Lúc này, thực hành công đoạn thu tín hiệu như chuỗi xung SE (FSE) để tạo ảnh cắt lớp khu vực nên khảo sát thì hầu như toàn bộ tín hiệu của các spin trong lòng mạch đã bị bão hòa bởi 2 sóng RF đảo ngược.
Kỹ thuật tạo ảnh black-blood thường được ứng dụng trong kiểm tra hình thái và cấu trúc của tim, trong đó xu hướng về kiểm tra cơ tim hơn là huyết động trong buồng tim. Hình ảnh black-blood chế tạo thông tin về hình thái như độ dày, tình trạng bệnh lý như viêm, huyết khối bám thành của cơ tim và gốc các huyết mạch lớn xuất phát từ tim. Việc lựa mua thời điểm thu tín hiệu (TR) theo điện tâm đồ (ECG) là vô cùng quan yếu vì di chuyển co bóp cơ tim sẽ ảnh hưởng trực tiếp tới chất lượng hình ảnh thu được.
a. Dark-blood thực hiện bằng Fast SE qua trục ngắn (short-axis) của tim tại thời điểm giữa tâm trương (mid diastole). Toàn bộ dòng chảy trong buồng tim đã bị bão hòa, chỉ hiển thị thông tin về cơ tim.
b. Dark-blood thực hiện bằng Fast SE qua trục ngắn của tim tại thời điểm cuối tâm trương (end diastole) – TR quá dài. Hình ảnh không rõ nét, nhòe do ảnh hưởng của chuyển động tiền tâm thu.
c. Dark-blood thực hiện bằng Fast SE qua trục ngắn của tim tại thời điểm tâm thu (systole) – TR quá ngắn. Hình ảnh không rõ nét, nhòe do ảnh hưởng của chuyển động tâm thu.
3.2. BRIGHT-BLOOD IMAGING
Hiệu ứng bright-blood diễn ra ngược lại sở hữu hiệu ứng black-blood đã kể ở trên. Thay vì dòng chảy bị bão hòa tín hiệu trong black-blood thì trong hiệu ứng bright-blood, cường độ tín hiệu của các cấu trúc có dòng chảy lại được thu nạp để tạo tương phản sở hữu cấu trúc ko sở hữu chiếc chảy xung quanh. Nguyên lý của hiệu ứng bright-blood được vận dụng phổ biến trong tạo ảnh cộng hưởng từ, bao gồm các chuỗi xung như TOF, PC-MRA, CE-MRA, có thể phối hợp hay không phối hợp sở hữu chất tương phản từ , có thể tạo ảnh 2D hay 3D.
Có một nhược điểm chung của các kỹ thuật tạo ảnh bright-blood không sử dụng thuốc đối quang quẻ từ là trong 1 số nếu thì hình ảnh thu được ko hoàn toàn trung thực mang hình ảnh thực tiễn do hiệu ứng dung tích riêng phần (partial volume effect) và tại những vị trí với cái chảy rối (turbulent flow).
Hiệu ứng thể tích riêng phần trong khoa học tạo ảnh bright-blood. Khi mặt phẳng cắt lớp vuông góc sở hữu thành mạch, hình ảnh lòng mạch thu được là chuẩn xác với tình trạng thực tế của mạch máu. Khi mặt phẳng cắt lớp ko vuông góc sở hữu trục thành mạch, hình ảnh lòng mạch thu được sẽ không hoàn toàn chính xác, thường nhỏ hơn so có tình trạng thực tế của mạch máu.
3.3. XUNG TOF
TOF (time-of-flight) là 1 trong các chuỗi xung chụp mạch cùng hưởng từ được ứng dụng đa dạng trong lâm sàng. Nói cách khác, trong một lớp cắt thì những phân tử vận động đến sẽ với cường độ tín hiệu cao hơn nhu mô bao quanh. Cường độ tín này phụ thuộc vào vị trí, tốc độ và hướng (góc) của chiếc chảy tới so với mặt phẳng lớp cắt mà ko phụ thuộc và chiều của loại chảy.
TOF được thực hành bằng phương pháp sử dụng một chuỗi xung GRE với TR ngắn (30-50ms) song song TE cũng được đảm bảo càng ngắn càng tốt. Góc lệch FA thường khoảng 20-400 trong tạo ảnh 3D TOF và > 50 trong tạo ảnh 2D TOF.
a. Nguyên lý tạo ảnh của TOF dựa theo hiệu ứng dòng chảy đến (inflow). Cường độ tín hiệu trong lòng mạch phụ thuộc vào góc và tốc độ dòng chảy đến so với mặt phẳng cắt lớp, không phụ thuộc vào chiều dòng chảy.
b. Nguyên lý tạo ra sự khác biệt giữa tín hiệu dòng chảy ĐM và TM trong TOF Sử dụng một sóng RF tiền bão hòa (presaturration) ở một lớp cắt trước lớp cắt cần tạo ảnh.
Cần nhớ lại trong chuỗi xung SE, hiện tượng black-blood xảy ra do hiệu ứng outflow (dòng chảy đi). Ngược lại với hiện tượng black-blood trong SE là hiện tượng bright-blood ứng dụng trong chuỗi xung TOF. Do các spin di chuyển trong lòng mạch nên không bị bão hòa bởi sóng RF nên các spin này có tín hiệu cao hơn. Trong khi đó, các spin của nhu mô bao quanh mạch do không di chuyển được nên sẽ bị bão hòa (saturation) do sự kích thích của các sóng RF có TR rất ngắn. Khi rơi vào trạng thái bão hòa, các spin này tạo ra tín hiệu rất yếu. Điều này cũng giải thích tại sao trong các chuỗi xung TOF, nhu mô xung quanh không trống tín hiệu hoàn toàn. Cường độ tín hiệu thu được từ TOF tuy phụ thuộc vào tốc độ, vị trí và hướng của dòng chảy nhưng lại không phụ thuộc vào chiều của dòng chảy. Như vậy để có thể tạo được ảnh riêng rẽ của dòng chảy động mạch hay dòng chảy tĩnh mạch, người ta tạo ra một vùng tiền bão hòa (regional presaturation) trước khi thu tín hiệu tại lớp cắt có các spin trong trạng thái bão hòa.
a. TOF 2D (ảnh gốc). Nhu mô não không mất tín hiệu hoàn toàn
b. TOF 2D (ảnh MIP) C. TOF 2D (ảnh MIP).
Những spin di chuyển đến rồi sau đó nhanh chóng đi ra khỏi mặt phẳng cắt lớp thì sẽ tăng tín hiệu, tín hiệu này sẽ giảm dần theo khoảng cách spin rời xa mặt phẳng cắt lớp. Những spin không di chuyển, chậm di chuyển khỏi mặt phẳng cắt lớp, hoặc di chuyển nhanh nhưng vẫn nằm trong mặt phẳng cắt lớp thì sẽ bị bão hòa, không hoặc ít tạo ra tín hiệu, đoạn mạch sẽ có màu tối đen (dark) trên hình ảnh cộng hưởng từ. Luôn nhớ một nguyên tắc, mặt cắt của TOF luôn phải không trùng với chiều dòng chảy của đoạn mạch muốn khảo sát.
Dựa trên những nguyên lý tạo ảnh đã nêu trên, để tránh có những sai lầm trong chẩn đoán, cần luôn phải lưu ý rằng hình ảnh mạch máu trong xung TOF chỉ là ước lượng gián tiếp qua dòng chảy trong lòng mạch. Nghĩa là đường kính của mạch máu đã bị giảm đi (underestimated), mức độ hẹp tắc có thể tăng lên (overestimated) so với thực tế. Độ tương phản của mạch máu có thể đã bị giảm đi do dòng chảy chậm hoặc đoạn mạch đó không thẳng.
